Community Health Worker Asthma Program

Community Health Workers at Public Health – Seattle & King County have been supporting families with asthma care in King County for over 20 years. The program has helped children and adults have more days without asthma symptoms, fewer emergency room visits, fewer missed days at school or work, and better quality of life.

Program benefits

  • Four customized sessions with a community health worker specializing in asthma care
  • A home environment assessment with a tailored plan to reduce asthma triggers in the home
  • Asthma trigger reduction supplies, including a HEPA air filter
  • Asthma education and resources

Eligibility

Anyone meeting the following criteria is eligible to participate in our program:

  • Consent to home, video, or telephone visits
  • Have poorly controlled, moderate, or persistent asthma
  • Be a resident of King County
  • Be at least 5 years old (younger children considered on a case-by-case basis)

Referrals

We invite healthcare providers and community service organizations to refer patients or clients to our program by submitting a quick online referral at the link below.

Alternatively, if you or someone you care about has asthma, you can complete the survey for yourself or on behalf of a loved one.

Module 1—Asthma: The Basics

In this video you will learn what asthma is, what effective control of asthma looks like, the difference between controller and reliever medicines and the importance of flu shots.

Module 2—Asthma Medicines

Learn the two types of inhaled asthma medicines: controller and rescue, or reliever medicines. We'll also cover the importance of taking medicines as prescribed, and ways to talk to your provider about your asthma medicines.

Module 3—Using Inhalers

Learn how to use different types of inhalers. What a spacer is and how to take care of your inhalers.

Module 4—Asthma Action Plan

Learn the importance of having an Asthma Action Plan and how to use an Asthma Action Plan.

Module 5—Asthma Warning Signs and What to Do

Early and Late warning signs for an asthma attack, what to do during an asthma attack and how colds can affect asthma.

Module 6—Asthma Triggers: Allergic

Learn how to describe what an allergic asthma trigger is. It covers dust mites, pets, cockroaches, rodents and mold.

Module 7—Asthma Triggers: Non-Allergic

Learn how to describe what a non-allergic asthma trigger is.

Module 8—Indoor air quality

In this video, you will learn about indoor air issues that affect asthma, products found in the Safer Cleaning Kit, cleaning techniques, and how to create a safe sleep zone.

Module 9—Outdoor air quality

You will learn how to reduce pollen exposure, different types of air pollution, and how to be prepared for wildfire smoke.

Module 10—Breathing exercises

In this module, you will learn how breathing exercises can be an important addition to helping with your asthma, different types breathing exercises and how and when to use breathing exercises.
