DCHS has four standard contracts, referred to as boilerplates, that are used to contract with agencies for services. Below is a brief description of each boilerplate and when it is used, as well as a link to the document.
Used when contracting with agencies when the amount of the contract is for $25,000 or more in service.
Used when contracting with a municipal corporation, school district, university, Harborview Medical Center, an educational services district or other public entity.
Used for HCD pass through for city projects, capital projects and Housing Finance Program projects.
Used when contracting with agencies that provide mental health and substance abuse services.
