What is a survey response form?

About 45 days before your survey is scheduled to begin, we will send you an official  notification of your survey requirement and a Survey Response Form (SRF). The SRF contains important information needed to set up your survey. Additionally, the SRF serves as a program update and provides a means of capturing any significant changes to your CTR program since your last formal Program Report.

Steps to completing your Survey Response Form

  1. Provide updated contact information and employee counts for your worksite
  2. Determine whether you will survey all of your employees or only those who are CTR-affected. This information will be used to calculate your response rate.
  3. Determine when you will begin your survey. We recommended that you do not survey the week following a holiday.
  4. Determine whether you will distribute paper survey or use the online survey system.
  5. Indicate whether or not you will be attending a FREE survey training session.
  6. Indicate whether or not there have been any updates since your last program report.
