Application For Services

Before you visit one of our offices, please take a few moments to review, the King County Veterans' Program application for services. This application presents many of the questions we will ask you when you arrive at our office.

It is not necessary for you to complete the application before your appointment, but if you have reviewed this information in advance, it will expedite your office visit. Feel free to print a copy of the application and bring it with you when you come to one of our offices.

The U.S. Department of Veterans Affairs Statement of Military Service (DD214) or the National Guard form (NG22) are the most important documents required of those seeking services at our offices. Please bring the appropriate documentation with you when you visit our office.
