2008 Annual Report FormsTable 1, 1A, 2, & 3 are Due Saturday, January 31, 2009Table 1 reports information about each unit (or bed) within your housing project, covering information on tenant households that resided in the housing during 2008. Tables 2, and 3 report aggregate information on all the units and tenant households served during 2008. Please pay special attention to the detailed instructions regarding each table. Contact the County staff listed below if you have any questions on how to complete the tables. Please complete a separate Excel Workbook for each project that received County funds, and indicate the housing project name on the table. Please use the Excel Workbook above. E-mail your completed report forms to KCAPR@kingcounty.gov no later than January 31, 2009. Earlier filing would be appreciated. (see right)
Tables 4, 4A & 4B are Due Tuesday, JUNE 30, 2009Tables 4, 4A and 4B report project income and expenses and project operating and maintenance reserve information. The *updated complete Excel Workbook (*workbook sent to us in January) . These tables must be emailed to us no later than June 30, 2009. For group home scattered site projects, it is permissible to combine operating statements and operating and replacement reserves for your submittal. If you intend to prepare a combined submittal, you must also be able to provide separate accounting for each physical location upon request.
In addition to Table 4, 4A and 4B the following submittals are also due no later than June 30, 2009. - Bank Statements or other documentation supporting reserve accounts
- Current Board of Directors list
- Program Assessment- please provide an Assessment of Program Effectiveness (a short write up about who was served by the housing, services provided and any major highlights or challenges related to the residents or the property)
- Annual Agency Report (the annual Board report that goes out to donors describing the activities of the agency)
- Insurance Certificate and Endorsements (if due) Please remember, agencies are required to provide a certificate of insurance naming King County Housing and Community Development as additional insured with all liability policies and loss payee with all property coverage policies. Both policies require endorsements from the respective underwriters
- Audited Financial Statements and/or OMB Circular A-133 audit (when available)
- Management plan change summary (submit only if changes in the past year)
- Capital Needs Assessment (Submit only if not previously submitted or if updated)
- Summary of tenant service changes (Submit only if changes in the past year)
- Marketing materials or state explaining approach to affirmative marketing
