Instructions for completing and mailing pet license application:

1. Download the pet license application form (PDF)
2. Fill out the application
3. Mail the following materials:

• completed application
• proof of pet spay/neuter or affidavit (if applicable)
• proof of age (if applying for senior citizen license)
• check made payable to King County Pet License
