• 504/ADA Self-Evaluation and Assurance of Compliance form will help your organization's or firm's programs and services, employment, and facilities to ensure they are accessible to people with disabilities. MS Word (130 KB DOC.); PDF (82 KB DOC)
• Affidavit and Certificate of Compliance (48KB DOC) form is a statement of compliance with King County Code (KCC) 12.16, Non-Discrimination and Affirmative Action in Employment.
• Bidding Instructions and Purchase Contract Conditions (52KB DOC) is a preprinted set of instructions and information that need to be considered by bidders and which are standard for all bids.
• Consultant Disclosure (60KB DOC) form is required from consultants for contracts over $2,500. This form is submitted to the Board of Ethics to determine whether there is any code of ethics or conflict of interest issues.
• Equal Benefits Substantial Compliance Authorization (286KB PDF) application for alternate compliance or "time delay".
• Equal Benefits Worksheet and Declaration (784KB PDF) form is required for all Contracts estimated to cost $25,000 or more.
• Personnel Inventory Report (PIR) (94KB DOC) lists employment data for businesses located in King County.
• Statement of Compliance (36KB DOC), Union or Employee Referral Agency Statement that the organization agrees to adhere to a policy of nondiscrimination and agrees to affirmatively cooperate in the implementation of the policies and provision of King County Code (KCC) 12.16.
• W-9, Request for Taxpayer Identification Number and Certification form is a reporting requirement of the IRS and King County must report income paid to suppliers.
