Sample consent forms for H1N1 vaccine. Federal law requires providers to offer patients receiving an H1N1 vaccination a copy of the H1N1 Vaccine Information Statement (VIS) for the formulation they are receiving. Signature for consent is not required by federal or state law, but may be by your facility or agency's risk management policy. The two examples of screening, consent, and documentation forms posted below--one from Washington Department of Health and one from Public Health - Seattle & King County--can be modified for use by your clinic or facility:
