Instructions to submit a Public Records Request

How to get a replacement Food Worker Permit

To obtain a replacement permit for your unexpired * Washington State food worker permit, fill out the form below.

  • *It is required to retake the class and test in full every time your permit expires.

  • If your unexpired permit is found in our records, you will be instructed to show up at any of our current class locations 30 minutes in advance of the class start time, show photo ID and pay $10.00. A new permit will then be issued to you.

PLEASE NOTE: We can only issue replacement permits if you took your last class and test through one of our locations listed below.

  • If you did not get your permit through us but elsewhere in King County, you likely got trained and tested by a private trainer who is registered to teach through the Washington Restaurant Association Education Foundation (WRAEF.) If this is the case, call Jennifer Dixon, the WRAEF representative who maintains enrollment records of everyone who got their permit through a private trainer in King County, toll-free at 1-877-695-9733, ext. 135 or by email at jdixon@WRAhome.com
  • If you did not get your permit through us but in another Washington State county, you will need to contact the county health department serving that area for assistance.
