Diabetes modules and handouts

Community Health Workers with the Steps to Health King County program provided home visits for Spanish, Vietnamese and English speaking clients with diabetes from September 2005 through August of 2008. Each enrolled client received baseline, multiple education and exit visits covering topics such as working with your provider, nutrition, foot care and physical activity.
