Risk Assessment Tool in Word format This assessment tool is intended to help individuals, such as clinicians who work with children on a regular basis assess the oral health of an individual. It is not a standardized assessment tool and is not meant to replace existing clinical evaluation methods. It is intended to help guide clinical decision-making toward preventive care for a child at risk of oral health problems. Additionally, it is intended to provide guidance to a child's parent or caregiver on preventive oral health practice.
