The following procedures should be followed for persons wishing to ask for copies of their personal treatment records.

  • If a person wants a copy of their treatment records, they should directly contact the agency or program where they received treatment.

  • If a person went to treatment at the Cedar Hills Addiction and Treatment Center that is now closed, send a request to:

    The forms you need to fill out to get a copy of your records will be mailed to you.

  • If a person wants a copy of their assessment from the King County Assessment Center that is now closed, send a request to:
