The FRC helps your family identify priorities and find and coordinate supports to match those priorities. Your FRC works with your family to coordinate:
If you do not know how to contact your FRC, or you don't already have a FRC contact Community Health Access Program (CHAP) (external), phone: 206-284-0331, or toll-free: 1-800-756-5437.
