How do I apply for Early Intervention - Child Development Supports for my infant or toddler?
A number of Early Intervention (EI) providers serve King County. Each of these agencies provides eligibility determination, service coordination and EI services and supports for children birth to three years old. An organization called Community Health Access Program (CHAP) (external) is your link to these centers. When you contact CHAP you will receive assistance in identifying the appropriate EI service provider with the resources and supports to address your child's needs. My child is 3 years old (or older) and has a disability. What services are available?
If your child has a developmental disability (e.g., mental retardation, Autism, cerebral palsy) or if you think your child might have a developmental disability, contact the State DDD office at 206-568-5700 or 1-800-314-3296 to see if you are eligible for services.
If you are not eligible for developmental disability services, contact the State Department of Social and Health Services at 1-877-568-5502 to see what assistance may be available to help you.
What is the difference between the State Division of Developmental Disabilities and the King County Developmental Disabilities Division?
The State Division of Developmental Disabilities (State DDD) is responsible for determining eligibility and authorizing paid services. State DDD currently provides the following services and programs: case management, Medicaid Personal Care, residential services, residential habilitation centers, family support, dental, mental health, Voluntary Placement Foster Care program, and the Medically Intensive program. State DDD administers services on a regional basis. Region 4 is the State DDD office that serves King County. The King County Developmental Disabilities Division (County DDD) is responsible for providing employment and day program services. County DDD currently provides the following services: Early Intervention/birth-to-three, employment, community access, housing, in-home family counseling, social and recreational activities, information and assistance, advocacy, homelessness projects, and program development/technical assistance.

How do I apply for services from the State Division of Developmental Disabilities?
Contact the State DDD office at 206-568-5700 or 1-800-314-3296 and ask for an application. Information will be taken over the phone. You will be asked to provide any available school or medical records that will assist in determining eligibility. Copies of other documents may be required. Be aware that eligibility cannot be determined until all of the necessary information is available. Download State DDD forms here (external). If you are a family with a child under three, application for State DDD eligibility determination for your child will occur as part of the Early Intervention Intake and Eligibility Process.
