Intake forms

If you complete and return an Intake Questionnaire, you have not filed a formal complaint. Office of Civil Rights staff will review your form, then contact you to finalize the process.

If you feel that you experienced discrimination, you can complete and submit a form to our office. Click on a PDF link below to open the appropriate questionnaire:

Please note: The Office of Civil Rights has authority to accept and investigate discrimination complaints only within unincorporated King County (or in King County Government).

When can complaints be filed with the Office of Civil Rights?

  • Employment - within 180 days of the alleged unfair employment practice (or within 180 days of when you became aware of it)
  • Housing - within 365 days after the occurrence or termination of the alleged unfair housing practice
  • Public Accommodations - within 180 days of the date of the alleged discrimination (or within 180 days of when you became aware of it)
  • Contracting - within 180 days of the time of the alleged unfair contracting practice (or within 180 days of when you became aware of it)
  • Disability Access - within 180 days of the alleged discriminatory act
  • Title VI - within 180 days of the alleged discriminatory act.
