Deferred Compensation Forms

There are no paper forms needed for enrolling in deferred compensation or conducting transactions, but you will need to use a paper form to designate beneficiaries. The Designation of Beneficiaries form is included in the enrollment packet available from T. Rowe Price. To request a packet and to enroll in deferred compensation, call 1-888-457-5770.

Once you're enrolled, you can conduct all transactions and get Designation of Beneficiary forms as needed through T. Rowe Price by phone or online. T. Rowe Price representatives are available to provide personal assistance over the phone weekdays, 4 a.m.-7 p.m. Pacific.

When you update your deferred compensation beneficiaries, remember to update your county insurance and state retirement beneficiaries, too.
