Step 1: Complete your Wellness Assessment Online between Jan 1 & Feb 28, 2013. Go Online 

OR: Complete your Wellness Assessment on Paper before Feb 28, 2013. Order by February 22 @ 206-684-1556 or email KC.benefits@kingcounty.gov. Between February 22 and 28 you can pick one up in person from the Benefits office at 401 5^th Avenue, Second Floor, Seattle, WA 98104. The filled out copy should be postmarked February 28 or before.     (If you missed the Feb. 28 deadline, be sure to take your wellness assessment by June 30 to earn Silver).

Step 2: Choose your Individual Action Plan and Complete it by June 30,2013.
