Healthy Incentives Community Meetings Credit Proceedure

Getting Healthy Incentives Credit for attending Community Weight Watchers meetings.

If you want to get credit toward gold for participating in a Weight Watchers™ community meeting to fulfill your individual action plan here’s how it works:

1. Pick a community meeting that works for you.

2. Pay for the meeting and retain your proof of payment. Your choices are:

a. Purchase a monthly pass through the Healthy IncentiveSM Weight Watchers @Work website (this will save you $3 per month). Present this pass to get into any community meeting. If you pay through the Healthy IncentiveSM website, no further proof of payment is needed.

b. Pay at the community meetings. Either submit a credit card receipt as proof of payment attached to your attendance sheet or, if you pay by cash or check – Submit send in a copy of the receipt Weight Watchers issues you (a sticker) as proof of payment. You will need to make a copy and send this proof to us with your attendance sheet.

C.  If you are a lifetime member, just send in proof of lifetime membership from Weight Watchers along with your attendance sheet.

3. Print the attendance form before your fist meeting. (It’s in the yellow box on the right side of the Healthy IncentivesSM website.)

4. Each time you attend a meeting, record the date and have your meeting leader sign the form.

5. After you have attended 10 meetings, submit your attendance sheet and proof of payment to Christi Masi at Christi.masi@kingcounty.gov, 401 5th Ave., Suite 200, Seattle, WA 98104, CNK-ES-0200.

Beginning in 2010, Weight Watchers at Work® will be part of the Healthy IncentivesSM Program and participation will qualify as your individual action plan.
