May 15: last day to begin your low-risk log

If Healthways notified you to keep a low-risk log as your Individual Action Plan this year, be sure to begin it no later than May 15. That will give you enough time – the 42 consecutive days – to complete your log by June 30 and earn gold for your out-of-pocket medical expenses in 2010.

If you cover a spouse or domestic partner under your county medical plan, he or she must also complete an action plan by June 30 to earn gold for your family.

To get started on your low-risk log, pick one of the logs under “Low-risk Individual Action Plans” on the right side of this page. Paper versions are also available.

To begin your log, you will first need to register using the access code that Healthways provided you. Once you begin, you can make entries to your log anytime during the 42-day period. Be sure to complete all entries to your log by the end of the 42nd day.
