A checklist for getting to gold with Healthy IncentivesSM individual action plans
If you’ve already earned gold for your 2012 out-of-pocket medical expenses, congratulations! But, if you’re still working on your individual action plan, these tips may help you. Remember the deadline to complete your individual action plan is July 1 to earn gold, the lowest level of out-of-pocket expenses for your 2012 medical coverage.
WebMD online individual action plan
Record three activities a week for 10 weeks before July 1.
Each week begins on Saturday and ends on Friday at 11:59 p.m.
Entries are recorded on the date you make them, even if the entry is for activity done a previous day or week.
After making an entry, visit your Rewards page to make sure that your entry was recorded. To visit your Rewards page, click on “Rewards (check status)” at the top of the page where you’ve recorded your entry.
When you meet a weekly goal, you will receive a confirmation e-mail and an update on your Rewards page the following Tuesday.
When you complete your 10th weekly goal, you will receive a confirmation e-mail and an update on your Rewards page by noon on the following Tuesday just as you have for meeting your previous weekly goals. Then, by noon on Tuesday one week later, you will receive confirmation of your gold status on your Rewards page.
WebMD paper individual action plan
Record three activities a week for 10 weeks in the individual action plan’s Diary.
Enter your name and Healthy Incentives ID on each page of the Diary.
Mail the Diary to WebMD with a postmark no later than July 1.
You will learn whether you successfully completed your paper individual action plan in a letter from WebMD in July. You may confirm that WebMD received your paper individual action plan. Wait at least one week from the date you mailed your action plan then call WebMD at 1-866-584-6813. (Enter the “1” when calling this number.)
Quit for Life® tobacco cessation
Complete 5 coaching calls by July 1.
Quit for Life® will notify WebMD when you’ve successfully completed 5 coaching calls.
Weight Watchers @ Work®
Attend 10 Weight Watchers @ Work® meetings at a county worksite by July 1.
Your Weight Watchers @ Work® team leader will notify the county when you’ve successfully attended 10 meetings.
Live Well Challenge
The Live Well Challenge ended April 10. The county is in the process of notifying all Live Well Challenge participants as to whether they successfully completed the Challenge.
