Only a few days left to complete wellness assessment

If you haven’t yet completed your wellness assessment this year, now’s the time—there are just a few more days before the March 4 deadline. Just log in at www.kchealthyincentives.com and click on the “Get Started” button under “Your Next Step.”

If you want a paper wellness assessment, call Benefits, Payroll and Retirement Operations at 206-684-1556 and plan on picking it up from the Benefits office on the second floor of The Chinook Building, 401 Fifth Ave., in downtown Seattle.

Complete the wellness assessment online by midnight, Friday, March 4, and you can move ahead with an individual action plan to complete by July 1 to earn gold. If you miss the March 4 deadline, you can still complete the wellness assessment by July 1 to earn silver for your 2012 medical out-of-pocket expenses.
