About a year ago my doctor told me that I have diabetes. I am trying really hard to control my diabetes through diet and exercise so I don’t have to take medication. It’s a lot to get used to. There’s a lot of information I have to keep track of and at times it can be confusing. I have to keep my regular appointments with my doctor and in between I have to measure my blood sugar and make sure I am getting in my walks.
My individual action plan choice
I’m doing the WebMD online program and I’m using the personal health record and health trackers. The personal health record helps be keep track of the times I have seen the doctor and what we talking about. It’s sometimes hard to remember all of the information she gives me when we meet so I just take 5 minutes to write it in my personal health record and I can go back to it at any time. I get an activity point for that. I also use the health trackers to keep track of my walks and my blood sugar. I like the graph feature because I can see that I am walking more and more steps and I can see my blood sugar over time.
Why this works for me
It has really helped me keep track of all of the information I need to know. I like the fact that I get credit toward gold for doing something I have to do anyway to manage my diabetes. And the tools are a really useful way for me and my doctor to track my blood sugar and exercise. I have even been able to see that when I get my walks in, my blood sugar is much better.

Anyone managing a chronic disease like heart disease or diabetes that requires keeping track of blood pressure, blood glucose, cholesterol or other biometric data.
Tips for success
Make sure you start your program before April 23rd if you need to skip any week for vacation, etc.
