Group Health’s Living Well with Chronic Conditions
Who would like it it: Anyone who has one or more chronic conditions and wants to learn new skills to manage their condition. The workshops take place in an interactive group setting of about 14 participants, so people who enjoy learning with others are most likely to like the format. Spouses, partners, and caregivers of people with chronic conditions are welcome to participate. Who can do it: Employees and spouses or domestic partners who are covered by King County medical benefits and have one or more chronic conditions. You do not need to be a Group Health member to participate. How it works: These six-week in-person, small-group workshops help people learn how to manage their chronic conditions and improve their quality of life. Participants learn to improve their health one step at a time by setting goals and then breaking the goals into small weekly action plans. Workshop participants learn and problem solve together. Two specially trained volunteer facilitators who have personal experience with a chronic condition lead the workshops. What you do to earn gold: Commit to actively participating in six 2 1/2 hour weekly sessions over a 6-week period. You must complete at least 4 of the 6 sessions to earn gold, but you'll benefit most by being present for all sessions. Workshop Locations and Times - All workshops are full.
