Coordinator Toolkit

Thank you for being a coordinator for the 2011 Onsite Flu Clinic Program. We anticipate delivering more than 3,500 flu shots to King County employees this year. We couldn't do it without you!

These resources have been developed to simplify your role as a flu clinic coordinator. However, you know your audience best - please feel free to add information or modify as you wish. Please try to keep the key messages: Don't let this happen to you. Don't let this happen to your loved ones.

Please let Reema know if there are additional materials you could use at your site. We're happy to try to create something to fit your needs.

Site-Specific Poster

Use this poster to advertise the flu clinic date(s) at your worksite. Plan to do this 2-3 weeks before your onsite clinic. If you do not have the ability to print this in color or as large as you'd like (it's formatted for 11x17 but can be printed 8.5 x 11), please call or email Reema at 206-263-6093.

Sample reminder emails

Use these to send to your building zz group announcing the flu clinics. They've been written assuming that only one clinic is happening on site so you may need to modify them accordingly. You'll see where these modifications may be necessary within each email - we've highlighted these areas in blue.

Consent form

Attach the consent form for to the reminder emails.  This form is for KingCare/Aetna and Group Health covered members.  If employees would like a reciept of service, they need to fill out a form onsite - they will recieve a copy of the form.  If they print this form and fill it out ahead of time, they will not receive a reciept.  

Note - the consent form may not print completely on letter sized paper.  Sometimes the bottom signature section gets cut off.  Check the form before filling it out.  You may need to print on legal sized paper.

Sign-up sheets and reminder cards

Use these if you intend to sign people up for time slots. Reminder cards are formatted for Avery business cards # 5376.
