Guardianship Forms

Below you will find a list of commonly used guardianship forms.  Guardianship forms must be completed in their entirety prior to your hearing.  Incomplete forms will cause delays and may possibly result in the need to reschedule your hearing.  For additional information regarding these forms, please contact the Family Law Facilitators Office at 206-296-9092 (Seattle), or 206-205-2526 (Kent).

One word of caution:  If you choose to use any of the forms provided here in MS Word format, please be advised that the appearance, page breaks, margins, fonts, and other aspects of the document that you prepare may be affected by the version of Word you are using.  Please be sure that the completed document conforms to all rules governing acceptable document format, particularly those stated in General Rule 14.  This includes but is not limited to the requirement that writing/printing shall appear on one side of the page only.

Training Video Requirement for Non-Certified Guardians Prior to Appointment

If you are seeking appointment as a non-certified guardian in a Type 4 guardianship, you are required to view the video:  “Instructions for Guardians,” pursuant to General Order #09-4-12051-7.  You may view the video online OR at one of the King County courthouses during the following times:

King County Courthouse (downtown Seattle)
Law Library, W-621
Monday through Friday, 9:00 a.m. to 5:30 p.m.

Maleng Regional Justice Center (Kent)
Room 3-D
Monday through Friday, 9:30 a.m. to 11:45 a.m.
Monday and Thursday 1:15 p.m. to 3:45 p.m

Once you have viewed the video, sign and file the Declaration of Proposed Guardian (Non-Certified), which is listed as Form #7 below.
