The navigation bar on the left side of this page provides links to various categories of forms maintained by Superior Court and the Superior Court Clerk's Office.  Please be advised that this directory is a work in progress and may not include all forms routinely used by the court or by individual judges.  In addition, Superior Court uses many of the Washington State Pattern Forms, particularly in the family law arena.  View the Family Law Handbook Marriage Edition or Domestic Partnership Edition maintained by the Washington Administrative Office for the Courts.  If you have questions regarding which forms to use, please contact the court.  Legal aid assistance and self-help information is available through the Northwest Justice Project and other legal resources.

The 'General Forms' in the table below are the most commonly used of the forms the court maintains.  Many of these forms can be used for multiple types of actions.  Before checking the individual subject area forms pages, please look to see whether the forms you need are included in the list below.  If you have questions about any of the forms below, please contact the Clerk's Office.
