All employees on taking military leave must also submit a copy of the orders for training or drill, with their absence request form.All employees who are activated for military service must provide Human Resources with the following signed forms prior to each activation. A copy of your activation orders.A copy of the completed Uniformed Services Leave Form for each activation.If you do not already have direct deposit, a direct deposit form must be submitted prior to activation.If you want to make any changes to your withdrawals, please submit a new W-4 form.If your activation will keep you from work during the next Wellness Assessment period, please submit a Wellness Assessment Exemption form to exempt you from the process.
