LCRLJ 11

Each new civil case filing, except small claims, petitions for domestic violence protection orders, anti-harassment orders and name changes, shall be accompanied by a Case Information Cover Sheet prepared and submitted by the plaintiff or plaintiff's attorney. The minimum requirements of this Case Information Cover Sheet shall be established by the Court.
